2023 Medical Plan Changes
- For 2023, Saratoga Hospital will offer two medical plan options:
- Premier Access Plus ($500)
- Premier Access ($1000)
- Medical Plans will continue to be administered by CDPHP
- Both medical plan options will offer an expanded Tier 1 network, where in most cases there will be no out of pocket costs. The Tier 1 network includes providers/facilities from:
- Saratoga Hospital
- Glens Falls Hospital
- Albany Medical Center
- Columbia Memorial Center
- The Visiting Nurses Association
- New CDPHP ID cards will be mailed to your home address.
2023 Prescription Drug Plan Changes
- The prescription drug plan will be administered by CVS Caremark
- Welcome kit including ID cards will be mailed to your home address
- For prescription drug coverage questions please call CVS Customer Care Line 1-877-281-5370
- It is not required that you go to a CVS Retail Pharmacy
- If you or a dependent are taking a Specialty Prescription Drug, you will receive information from CVS Caremark and PrudentRx.
- Mail Order Available
- 90-Day Supply available at the cost of 2 1/2 times the copay
- 90-Day Supply available at the cost of 2 1/2 times the copay
